We have a simple process that tells us if you are eligible for COASTAL support, and gives us some basic information about you. The first step in this process is to complete a referral form, which you can download using the link at the bottom of this page.
Who can refer you?
Anyone who is supporting you directly, this could be a Social Worker, Support Worker, Employment Advisor or Young Person’s Advisor (YPA).
You can also refer yourself to the project. If you need help to complete the referral form please contact us and we can arrange for a member of staff to assist you.
What happens next?
Once we have your information we will be able to make a decision about how we can support you. There are two teams within COASTAL that offer different types of support, Person Centred Planning, and the Employment Team.
You will be contacted by a member of the support staff, who you will work with directly. Each team also has a Coordinator, who you will also be in contact with, and will help you to manage the support you receive.
